TENS, muscle stimulator and interferential electrodes are supplied in more than one size because electrode area decides current density. The same output spread over a larger pad feels gentler; concentrated into a small pad it feels sharper and works a tighter area.

Choosing size is a clinical decision rather than a preference. Larger electrodes suit broad areas such as the lumbar spine, quadriceps and hamstrings. Smaller electrodes suit the forearm, calf, foot and the muscles around the shoulder, where an oversized pad would recruit muscles you did not intend to treat.
